Insurance information

Being a specialty practice, we do not accept direct payments (assignment) from insurance companies. Hence, you are fully responsible for all fees charged by this office regardless of your insurance coverage when treatment is rendered. However, we can submit your insurance claim form electronically to your insurance company to assist you in being reimbursed. Please bring your insurance information with you to your appointment.
